Dear Ms. Okafor,
This letter confirms that your employment with Cedarline Health Systems will end effective the date noted below. Your employment is at-will, and this decision was reached following a workforce restructuring within the Medical-Surgical service line. This action is not a reflection of any misconduct on your part.

Your final scheduled shift is the evening of June 30. Badge access ends at 11:59 PM the same day.
We recognize the dedication you have shown to your patients and colleagues over the past six years, and we are committed to making this transition as clear and supported as possible. The information below outlines your final pay, your benefits, and the items we will need returned.
What Happens Next
Final Pay
Your final paycheck, including 92.5 hours of accrued, unused PTO, will be direct-deposited on July 10, 2026, the next regular pay date following your last day.
Health Benefits & COBRA
Your medical, dental, and vision coverage continues through July 31, 2026. A COBRA continuation election packet will be mailed to your home address within 14 days, with a 60-day election window.
Retirement Plan
Your 403(b) account remains yours. Empower Retirement will contact you regarding rollover or distribution options. Vested contributions are unaffected by this separation.
Return of Property
Please return your facility badge, parking transponder, and any clinical reference materials to the HR office on or before your last day. A return checklist is attached.
As a reminder, your continuing obligations regarding patient privacy under HIPAA and the confidentiality of Cedarline proprietary information survive the end of your employment, as set out in the Employee Confidentiality Agreement you signed at hire.
