Safety

Operated Forklift #7 to move three pallets without completing the required pre-operation inspection checklist. A floor audit later found the horn was non-functional on that unit.

Impact: Documented safety-protocol breach; unit pulled from service for repair.
4Expected Corrective Action

Arrive on time for every scheduled shift. Be clocked in and staged at the assigned dock by 3:00 PM, with advance notice to the shift lead for any unavoidable delay.

Complete the pre-operation forklift inspection in full before moving any load, and report any equipment defect immediately rather than operating the unit.

Sustained, consistent compliance is expected going forward. There is no fixed review date attached to this warning; your supervisor will continue normal shift oversight.

Consequences of Continued Conduct

Failure to meet these expectations, or any further attendance or safety violation, will result in further disciplinary action up to and including suspension or termination of employment, consistent with the company's progressive discipline policy.
