I authorize Cedar Valley Property Management to initiate electronic debit (ACH) entries from the bank account identified above for the amount and on the schedule indicated, and to credit corrections if needed. I certify I am an authorized signer on this account.
This authorization remains in full force until Cedar Valley receives written notice of its termination at least 5 business days before the next scheduled debit. Revoking it does not cancel charges already owed.
