Medications, Conditions, or Notes for Trip Leaders
Parental affirmation. I grant permission for my student to participate in the Oregon Coast Aquarium field study on May 17, 2024. I understand the trip involves travel by charter bus, coastal tidepool activity, and supervised entry into cold Pacific water while wearing a fitted wetsuit. I authorize Cascade Ridge faculty to seek emergency medical care on my student's behalf, and I release the district from liability for ordinary risks associated with coastal field activity.
