It is my privilege to formally recognize your team as the recipient of the Q1 Patient Safety Excellence Award. This is the highest honor our institution bestows for clinical performance, and you have earned it unequivocally.
Between January 1 and March 31, your team administered medications to patients across the 3rd, 4th, and 5th floor units without a single documented error. In an era when the national benchmark hovers near one error per every several hundred administrations, what you accomplished is not a statistic. It is a discipline — the kind that is built shift by shift, checklist by checklist, and sustained through the quietest and most demanding hours of the day.

Errors
All Three Units
Behind every one of those administrations was a patient — often asleep, often frightened, often too unwell to advocate for themselves. You advocated for them. You double-checked the dose after a 10-hour shift. You questioned the order that didn't quite fit. You caught what the system missed. That is nursing at its finest, and it deserves to be named aloud.
