Service Waiver & Consent

Please review and sign before your treatment. This release covers the service named below and your aftercare.

I am receiving the service noted below at Maison Lumière Salon & Skin Studio (the “Studio”). In consideration for the service, I, the undersigned Client, acknowledge and agree to the following:

Acknowledgment of risk. I understand that chemical, color, lash, waxing, and skin treatments carry inherent risks — including skin irritation, redness, allergic or sensitivity reactions, chemical burns, breakage, staining, eye irritation, and uneven or unexpected results. Individual results vary.

Release of liability. I release the Studio and its technicians from claims for any reaction, injury, or dissatisfaction arising from the service, provided it was performed with reasonable care, to the fullest extent permitted by law.

Aftercare. I agree to follow the aftercare instructions I am given and to contact the Studio promptly if I notice an adverse reaction.
Allergy & sensitivity disclosure

I confirm I have disclosed any known allergies, skin sensitivities, recent treatments, medications, or pregnancy that may affect my service, and I understand a patch test may be recommended.

Sensitive to fragranced products; no known chemical allergies
