Every player must have this release signed by a parent or guardian before the first practice.
In consideration for the named player being allowed to participate in programs run by Riverside Youth Soccer League (the “League”), I, the undersigned parent or guardian (the “Participant” signing on the player's behalf), acknowledge and agree to the following:

Assumption of risk. I understand that soccer is a contact sport with inherent risks — including collisions with other players, falls, sprains and fractures, contact with the ball or goalposts, heat illness, and concussion or other head injury. I accept these risks on the player's behalf.

Release of liability. I release and discharge the League, its coaches, volunteers, and officials from all claims for injury, loss, or damage arising out of the player's participation in practices, games, and league events, to the fullest extent permitted by law.

Medical consent & hold harmless. If I cannot be reached, I authorize League staff to arrange emergency medical care for the player, and I agree to hold the League harmless for any claim arising out of participation.
