This letter serves as formal documentation of two separate safety violations occurring within a thirty-day period. It represents the final disciplinary step before termination of employment.
Dear Mr. Alvarado,
IronRidge Construction Group enforces Personal Protective Equipment requirements in full alignment with OSHA standards 29 CFR 1926.95 through 1926.102, as well as the site-specific Safety Plan you signed upon onboarding. Compliance is not optional; PPE is the single most important safeguard between our crew and serious injury on an active build.
Within the past thirty days, two documented incidents of non-compliance have been recorded. Both were directly observed by site leadership and logged in the daily safety report.
Observed operating within the swing radius of Tower Crane #2 without a hard hat during a scheduled steel lift. You were verbally counseled on-site and a first written notice was issued the following morning.
Observed within six feet of an open perimeter edge with a fall-arrest harness worn but not anchored to the approved tie-off point. Work was halted and a toolbox stand-down was called for the remainder of the shift.
Per IronRidge Progressive Discipline Policy §4.2 and OSHA-aligned site conduct rules, this is the final corrective step available. A third documented violation of any category — hard hat, harness, eye protection, footwear, or high-visibility apparel — will end your employment effective the same shift, without further warning.
