Please read each section before your appointment. Your artist will go over it with you and answer any questions.

The consent

I am voluntarily getting a tattoo at Ironwood Tattoo Co. I am 18 years of age or older and not under the influence of alcohol or drugs.

I have informed my artist of any medical conditions, allergies, or medications that may affect the procedure or healing.

Risk acknowledgment

I understand a tattoo is permanent and that color, placement, and healed appearance can vary.
I understand the risks include infection, allergic reaction, swelling, scarring, and possible bloodborne exposure.
I have reviewed and approved the final design, size, and placement before work begins.

Aftercare

I have received aftercare instructions and understand that proper healing is my responsibility. Ironwood Tattoo Co. is not liable for results caused by failure to follow aftercare or by my own medical conditions.
