I understand that participation in soccer involves the risk of injury, including the risk of concussion and other serious injury. I have reviewed the league's concussion information sheet and agree that my child will be removed from play and evaluated if a concussion is suspected, returning only with written medical clearance.
I release Riverside Youth Soccer, its coaches, officials, and volunteers from liability for injuries sustained during normal participation, except in cases of gross negligence, and I authorize league staff to obtain emergency medical care for my child if I cannot be reached. My child and I agree to abide by the league's Code of Conduct for players, parents, and spectators.
