Participation

I give permission for my child to participate in {{ org_short }} practices, matches, and team activities for the season above, including travel to away matches supervised by the club's coaches and approved volunteers.

Player safety & risk

I understand soccer involves normal physical activity and the ordinary risk of minor sports injuries. The club follows its concussion and heat-safety protocols, and coaches are first-aid trained. I have shared any condition the club should know about below.

Emergency medical authorization

If my child is injured and I cannot be reached, I authorize the club's staff to seek and consent to emergency medical care on my behalf, and I will be notified as soon as possible.
