I am referring this patient for rheumatology evaluation of suspected inflammatory arthritis.

Reason for Referral

Persistent joint pain and morning stiffness in bilateral hands and wrists for 8+ weeks. Elevated ESR and CRP on recent labs. Rule out rheumatoid arthritis.

Clinical History

38-year-old patient presenting with 8 weeks of bilateral hand/wrist pain and morning stiffness lasting >1 hour. Physical exam shows mild swelling of MCP joints bilaterally. ESR 42 mm/hr (elevated), CRP 2.8 mg/dL (elevated), RF pending.

Current Medications

Sertraline 50mg daily, Naproxen 500mg BID PRN
