How to make a video release form

Any time someone appears in footage you plan to broadcast, stream, or promote, you need their permission in writing. A video release — also called an appearance release — grants the production the right to record and distribute that person's appearance, voice, and likeness.

What to put on the form

• Participant: name, on-screen role, email, and phone.
• Production: the project or series, the producer, and the recording date.
• Usage rights: broadcast/TV, streaming/VOD, promotional and trailers, and social media.
• Terms: territory and term, so distribution scope is unambiguous.
• Sign-off: participant signature and date, plus a parent or guardian line required only if the participant is under 18.

Send it and get it signed
